Gilman Healthcare Center is a 99-bed skilled nursing and rehabilitation facility located at the crossroads of Gilman just off Interstate 57. We are committed to achieving excellence in both nursing and rehabilitative services. Our interdisciplinary approach focuses on functional outcomes and successful discharges to the communities of Iroquois, Champaign, Ford, Vermilion, and Kankakee counties. Since opening its doors in 1969, the facility has grown from a 49-bed facility to a 99-bed, 100% Medicare/Medicaid Certified facility. For over 40 years, Gilman Healthcare Center has cared for our community members who need 24-hour skilled nursing care and rehabilitation, returning loved ones to their homes. In recent years it has added a beautiful, massive dining room and a state-of-the-art therapy gym.

Meals On Wheels

Our Meals on Wheels Program is available to our local community. Any nearby area member can have a hot meal delivered to their door for a small fee. The program, which is available five days a week, seeks to ensure that everyone has a great hot meal at a reasonable price available to them.

Crossroads

As the Developmentally Disabled (DD) and those with an Intellectual Disability (ID) population age, there are times that they will require skilled care and rehabilitation therapy. Gilman Healthcare Center’s Crossroads Program offers the DD and ID population an individualized care program for those who need short-term rehabilitation therapy. Crossroads offers residents quality and excellence in physical, occupational, and speech therapy in a nursing environment and meets the unique needs of the DD and ID resident.

The Crossroads Program ensures that the short-term resident returns home to their prior residence. While a resident of Gilman Healthcare Center’s Crossroads Program, residents can expect the continuation of life skill training they may receive as part of the overall supportive environment. Crossroads helps bridge the gap between aging and developmental disabilities.

In respect of our residents, we wish to follow Rosa’s Law. Rosa’s Law, S.2781 became federal law on October 5, 2010, removing the term “mental retardation” and “mentally retarded” from federal documents and replacing them with “individual with an intellectual disability” and “intellectual disability.

Emergency Preparedness

Gilman Healthcare Center maintains emergency preparedness plans and policies, which are available for review by residents, families, staff, and other resident representatives.
